Course Description:

This course introduces an evidence-based approach to creating a patient-centered medication management care plan in home hospice.  Key components will provide guidance on how to provide support to family caregivers who manage home hospice medications, identifying potentially unnecessary medications, and how to have conversations with patient, family members, and other clinical providers about deprescribing.

Instructions for Successful Completion and Credit

Participants have 30 days to complete the module after registration. To successfully complete the program and receive a certificate, participants must 1) read the learning objectives and faculty disclosures; 2) study the educational activity; and 3) complete the evaluation form. After successfully completing this evaluation, you will be sent an email with a link that will allow you to view and then print your CNE certificate immediately.
